This position will take part in Elevance Healths hybrid workforce strategy which includes virtual work and 3-4 days physically in office per week.
Associates are required to live within a reasonable proximity (50-mile radius and a 1-hour commute) to either one of our Elevance Health major office (PulsePoint) locations in Florida listed above. Elevance Health supports a hybrid workplace model with PulsePoint sites used for collaboration, community, and connection.
The Manager of Health Promotion Outreach and Services is responsible for managing and coordinating comprehensive SDOH programs targeted at Whole Health Promotion activities that enhance the value of managed care products and services for the Florida Pathways program. This includes the development of selected health-related policies and services, and design and management of SDOH programs that support Pathways’ program goals.
How You Will Make an Impact
Primary duties may include, but are not limited to:
Coordinates the development and implementation of the Pathways Program and all service offerings under the program.
Acts as a liaison to quality management, health care management services, and the disease management program for the coordination of joint activities.
Tracking of data and reporting of Pathways Program outcomes.
Researches and recommends management strategies that can enhance ability to provide members and providers with cutting edge services and programs.
Identifies and recommends activities and services that promote member compliance, participation, and retention.
May also manage community events, member services (complaints, claims reimbursement), etc.
Assures compliance with contract requirements and ensures member and provider materials are State approved.
Hires, trains, coaches, counsels, and evaluates performance of direct reports.
Minimum Requirements:
Requires a BA/BS degree in Health Promotion, Health Education or related field and a minimum of 6 years of experience in a related field including management experience; or any combination of education and experience which would provide an equivalent background.
Preferred Skills, Capabilities, & Experiences:
Data tracking and outcome reporting.
PowerPoint development and presentation skills.
Knowledgeable about Whole Health and SDOH interventions.
Relationship building with key stakeholders.
Strong team leadership skills.

Elevance Health operates in a Hybrid Workforce Strategy.
