Job Description
Youth Services Coordinator
Reports To: Director of Programs and Services
Hours of Work: Hourly, nonexempt, 38 hours per week – hours may vary as needed

POSITION SUMMARY:Provides opportunities for young people with significant disabilities to develop the skills needed to become self-reliant, contributing members of the community through involvement in age appropriate, integrated, community-based activities.
Increase access to free and appropriate public education services for young people with disabilities through the provision of Individualized Education Plan advocacy and support services.
Assist young people with disabilities to acquire the goals on the Youth Services Transition Checklist which are guidelines that children and youth need to achieve before entering adult life.
EXPECTED RESULTS:The annual efforts of this position should result in:
1.70 children in inclusive community programs,
2.90 unduplicated parent and young people attend at least 1 of 5 planned activities,
3.90 parents receive educational advocacy assistance with IEP/ITPs,
4.60 active consumers served per year,
5.85%of youth involved in inclusive community programs who complete the program will indicate that services benefited the participants emotional development in at least one of the targeted areas, as reported by youth and/or parents,
6.80%of community/social participation goals will be met by program participants,
7.70%percent of parents will report that they were successful in securing needed services in their childs IEP/ITP,
8.70%of individualized educational goals will be met by program participants,
9.70% of socialization/networking activities participants will indicate that they initiated or strengthened a peer relationship,
10.50 unduplicated individuals will receive group advocacy training in 1 of 2 training activities.
