process of an in-home caregiver for a fellow community member. Have you

8:30a-10:30a/10p-12a/every other weekend 8:30a-10:30a Hiring for all
shifts! Flexible scheduling options! Wage: $20 per hour Tasks include
but are not limited to: Hands-on personal care Toileting Catheter Care
Bowel Program Transfers using a transfer board. Light Housekeeping Range
of Motion exercises Med Reminders Ideal Candidate: Must be 18 years of
age. At least 1 year of professional caregiving experience in a hospital
or skilled nursing facility CNA Required Must pass a caregiver
background check. Reliable transportation and backup transportation plan
Dependable, energetic, self-motivated & well organized Must be able to
lift 50# safely and comfortably. Community Horizons offers support for
individuals enrolled in self-directed programs through Managed Care
Organizations or IRIS (Include, Respect, I Self-direct) throughout
Wisconsin. Self-direction gives program participants the choice over who
provides services and how they receive them. Community Horizons is not
the employer on record, members make all hiring decisions with the
involvement of a third-party fiscal agent responsible for payroll.
