JOIN OUR TEAM AND MAKE A DIFFERENCE!“Action Inc is a non-profit organization dedicated to developing pathways out of poverty in Southwest Montana through innovative, community-based, collaborative approaches.” Position Overview: We are currently seeking an energetic Youth Service Advocate to join our team. Through a respectful, constructive, and energetic style guided by the core values of the organization, the Advocate plays an integral role in delivering support services to youth and young adults. The Advocate works with youth and young adults to learn skills that promote independence and success. Duties and Responsibilities: Through a coordinated community approach, the Youth Service Advocate will identify and quickly connect eligible youth and young adults to programs and resources that help overcome barriers to living independently. This position provides case management and coordination of individualized service plans based upon identified goals and needs. The Advocate works in a team environment to integrate services in a six-county service area, work collaboratively on program development, compliance, and utilization of grant funds. Duties include but are not limited to the following: •Works with a collaborative team at Action Inc. to determine eligibility for supports in accordance with applicable federal, state, and local regulations; determines funding sources will be deployed.•Responsible for conducting formal assessments to what supports are needed.•Develops, monitors, and updates youth-centered goal plans with program participants, assisting them to set and prioritize short and long-range goals.•Provides supportive services and/or referrals as needed that align with policy guidelines.•Completes timely, accurate data entry and progress notes in required data reporting systems and maintains participant files.•Conducts outreach to youth and young adults and connects participants to youth serving providers in the service area. Travel throughout the six-county service area is required.•Builds and maintains effective collaborative relationships with other involved agencies. •Participates in team meetings with supervisors, other staff, and agencies. •Attends trainings regularly as required by funding source or as required by supervisor.•Exemplifies behaviors in the workplace that are consistent with the core values and mission of Action Inc. and that encourage trust with program participants.•Performs other duties as assigned which are consistent with the general responsibilities of the position.Qualifications: Bachelor’s Degree in the Human Service field and 2 years case management or related experience with disabled or otherwise disadvantaged population is preferred. Ability to deliver services to youth and young adults with unique backgrounds and maintain a safe, open, and empathetic environment. Must possess knowledge of community resources, the ability to effectively conduct group meetings and trainings and effective written, oral and interpersonal communication skills. Must have the ability to communicate, comprehend and perform functions from written and oral instructions. Must have ability to exhibit mature judgment. Must be able to operate common office equipment and have computer skills. Will consider a combination of education and experience.Certificates, Licenses, Registrations: Must be bondable and insurable. Must possess a valid Driver’s License.
