ASSISTANT DIRECTOR OF HEALTH HOMEDevelopmental Disabilities Hybrid (Kansas City and surrounding areas)
Job overview:
The Assistant Director of Health Home will oversee and supervise the daily operations of the Health Home Team. The position is responsible for coordinating and tracking Health Home developmentally disabled clients. In this role, you will supervise, coordinate and facilitate health education groups and activities. Lastly, the position will serve as the Cyber Access Practice Administrator.

What youll get to do:
Oversee and supervise the daily operation of the Health Home Team
Responsible for coordination/tracking of Health Home enrollees (new enrollees, discharges, transfers)
Supervise Nurse Care Managers and assign NCM caseloads
Coordinate and/or facilitates health education groups and/or activities for clients/staff/community.
Responsible for reviewing, completing and submitting, in a timely manner, the monthly Attestation report in CyberAccess.
Serve as a Cyber Access Practice Administrator responsible for maintaining user accounts and monitoring agency usage and prescriber reports.
Coordinate management of HIT tools and initiatives, including CyberAccess usage, CMT reports and hospital admission/discharges.
Oversee and/or facilitates staff training on Healthcare Home.
Participate in monthly TA calls for Health Home Directors and attends state required meetings and trainings.
Oversees Quality Assurance and Quality Improvement for HealthHome.
Qualifications:
BSN or MSN
Registered Nurse
