System Coordinator

POSITION: System Coordinator Coalition for the Unhoused
DUTIES:
1. Collaboration and Networking
– Build and maintain strong relationships with key stakeholders across both
counties, including government agencies, service providers, community
agencies, and housing providers.
– Foster a collaborative environment to facilitate effective communication and
resource sharing.
– Liaise between various partners and teams to establish shared knowledge of
available services and resources.
2. Project Management and Implementation
– Lead and oversee projects aimed at addressing homelessness and improving the
effectiveness of homeless services systems.
– Collaborate with diverse stakeholders – including people with lived
experience of homelessness – to plan, execute, and monitor initiatives and
projects using continuous quality improvement methodologies
– Manage project timelines, tasks, and deliverables.
3. Change Management and System Improvement:
– Lead change cycles and identify areas for improvement using qualitative and
quantitative data
– Develop and implement evidence-based practices and strategies to enhance
service delivery and outcomes.
– Identify gaps, inefficiencies, and opportunities for improvement and
develop action plans to address them.
4. Data Analysis and Reporting:
– Leverage data analysis to inform decision-making and assess the impact of
system changes.
– Create and maintain systems to track and measure project performance.
– Generate reports and dashboards to communicate progress and outcomes.
5. Coordinated Entry/Case Conferencing and Follow-Up:
– Facilitate and/or co-design Coordinated Entry System and Case Conferencing
meetings
– Utilize case conferencing best practices such as bucketing client phases,
using data to identify process bottlenecks, and establishing accountability
structures
– Implement follow-up procedures to track client progress and outcomes.
6. Capacity Building and Training:
– Pinpoint and amplify training and capacity-building necessities for agencies
and key players.
– Spearhead transformative training sessions, workshops, and seminars.
REQUIREMENTS:
– Bachelor s degree in social work, public administration, community
planning, or a related field is preferred.
– At least 3 years of experience in project management, systems
coordination, or homelessness services, with a focus on community-based
initiatives.
– Demonstrated experience in facilitating multi-stakeholder collaboration,
change management, and continuous quality improvement methodologies.
– Knowledge of data analysis and reporting tools to inform decision-making and
track progress.
– Experience working with government agencies, non-profit organizations,
and community-based services.
OTHER REQUIREMENTS:
– Valid drivers license and reliable transportation for travel within Las
Animas and Huerfano counties.
– Ability to pass a background check.
– Strong verbal and written communication skills.
– Proficiency with Microsoft Office, Google Workspace, and project
management software.
– Flexibility in schedule, with the ability to work occasional evenings and
weekends based on project needs

WAGE: The salary range for this position is $70,000 to $80,000,
depending on qualifications and experience.
