Description:
DCM will be overseeing a team of about 15 employees– 4 MSW/LCSW/LMFTs, 2 managers with teams of case managers and community health workers.
DCM time will be distributed anywhere in the range of 50 to 60 percent of time on administrative duties and the remaining time as a provider.
Collaborate with other professionals to evaluate patients medical or physical condition and to assess client needs.
Utilize consultation data and social work experience to plan and coordinate client or patient care and rehabilitation, following through to ensure service efficacy.
Monitor, evaluate, and record client progress according to measurable goals described in treatment and care plan.
Plan discharge from care facility to home or other care facility.
Modify treatment plans to comply with changes in clients status.

Additional Skills & Qualifications:
LCSW/MSW with 5 to 8 years’ experience in a senior level with at least three years of Behavioral Health admin experience
Must be bilingual (English/Spanish)
Will be a provider also with a patient load
Leadership / administrative skills required
Schedule: Fulltime M-F 8am-5pm ON SITE
