Outreach Worker/Case Manager

Outreach Regularly conduct outreach in areas known to have a high
concentration of homeless individuals, including parks, encampments, and
other public spaces. Establish trust with individuals experiencing
homelessness through consistent engagement, empathy, and non-judgmental
support. Assess clients? needs and vulnerabilities, including health,
safety, substance use, and housing, and develop individualized plans to
address these needs. Provide referrals to shelters, food resources,
medical services, substance use treatment, mental health support, and
other relevant community services. Attend individual supervision, staff
and community meetings Respond to crises, including mental health
emergencies, substance overdoses, and safety concerns, by providing
immediate support and intervention. Maintain accurate and timely
documentation of outreach activities, client interactions, and service
referrals in accordance with organizational policies. Work closely with
local government agencies, shelters, hospitals, and law enforcement to
coordinate services and address issues related to homelessness in the
community. Ensure consistent follow-up with clients to monitor progress,
provide ongoing support, and encourage continued engagement with
services. Shower Van Safely operate and drive the shower van to
predetermined locations in the community. Follow all local traffic laws
and safety regulations. Ensure the van is adequately stocked with
supplies (towels, soap, shampoo, etc.) before each shift. Set up the
mobile shower unit upon arrival at each location. Maintain a clean,
safe, and welcoming environment for clients by regularly cleaning and
sanitizing the shower stalls after each use. Monitor water levels,
sanitation, and waste management systems within the van. Troubleshoot
minor maintenance issues with the van or shower equipment and report any
larger issues to management. Adhere to all health and safety guidelines,
including proper sanitization of the van and showers. Ensure clients
follow health protocols while using the facilities. Respond calmly and
effectively to any crises or emergencies that arise during operations.
Other Demonstrate strong and clear communication both written and
verbal. Abide by handbook rules, policies and code of conduct. Support
operations within the Engagement Center on a daily basis Perform other
related duties as assigned. Education, Abilities, Competencies and
Experience Required High School Diploma Driver license required Position
requires ability to work with homeless sub-populations, including
individuals and families with substance abuse, mental illness,
developmental disabilities, veterans, elderly and people with multiple
diagnoses One year working with street outreach, homelessness, or human
services setting required Experience working with clients with mental
illness and substance use disorders Ability to communicate effectively
orally and in writing with special attention to details Demonstrated
ability to cultivate and maintain positive working relationships with
other agencies Proficient computer skills with email, data entry, word
processing and spreadsheet Supervisory Responsibility This position has
no supervisory responsibilities. Work Environment This position is
primarily off base and will require field work under all-weather
conditions. Physical Demands Ability to walk up to three miles in a city
setting, rural areas, and in forestry. Ability to carry a backpack with
supplies weighing up to 20 lbs. with supplies needed.
