Job Details Licensed Practical Nurse: A Licensed Practical Nurse (LPN) is responsible for assisting in the safe treatment and care of patients in accordance with established policies and procedures and age of patients served on the unit. Responsibilities include: Responds appropriately to the special needs of each group served specific to age, language, gender, culture, religion/spirituality, economic background, education or literacy, developmental or cognitive disabilities, physical and developmental disabilities. Employee aids in the orientation of new employees. Practices within the LPN scope of practice. Identifies and reports change in patient status to RN in a timely manner. Performs and is knowledgeable of vital signs, medication administration, monitoring lab values, care of patient pre/post procedure, geriatric competency, pediatric competency (when required), hospice care, pain management, patient safety, restraints, isolation precautions/PPE, admission and discharge of patients. Uses and is knowledgeable in the care and maintenance of gastrostomy tubes and foley catheters. Employee is able to provide written and verbal educational material to patients/families, and documents education as per policy. Troubleshoot/assemble equipment as needed. Participates in Code 1000/Code D/Code Red as directed. Updates/implements patient plans of care appropriately. Transcribes and documents physician orders accurately using computer order entry system. Accurately reconciles medications upon admission and discharge of patient. Is familiar with and participates in hospital Performance Improvement. Responds to patient needs and call bells in a timely manner. Advocates for patient safety and quality of life Keeps medication and treatment carts/room neat clean and orderly. Monitors and implements IV therapy as per Physician Orders. Requirements: Graduate of an accredited school of practical nursing. Licensed as an LPN in New York State Licensed as an LPN in New York State Basic computer skills Previous experience preferred. Knowledge of safety and infection control policies. Knowledge of computer systems and/or prior clerical experience preferred, BLS Certified. Excellent verbal and listening skills required for communicating clearly and effectively with others. Physical Requirements Position is moderate and requires frequent standing, bending pushing/pulling interspersed with walking to and from various locations. Ability to bend, stretch, reach, handle/finger delicate items required. Ability to carry items up to 5 lbs. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job and it does not include the value of benefits.
