Description

Effectively supports ongoing and collaborative communication amongst all Nursing staff and is responsible for ensuring a trauma informed care environment.Provides medical services to all individuals receiving health care within CCFCS programs. Works collaboratively with patients and staff, a range of other providers and natural supports to facilitate client health and wellness stability.

Essential Duties and Responsibilities

Provides comprehensive health care of patients

Identifies immediate and long-term health care needs.

Makes appropriate referrals and recommendations based on findings of the health assessments.

Assists clients in identifying local resources.

Communicates and collaborates with appropriate Agency staff, Primary Health care providers, community justice agencies, and related community organizations to assure coordination of client care and services.

Facilitate and provide training, oversight, and supervision of all Nursing staff.

Coordinates and oversees job-specific, new hire Registered Nursing orientation utilizing Medical Team Orientation checklist appropriate to each program site

Provide training, resources, and support to RN, facilitating the supervision of Medical Liaisons.

Supervise Registered Nurses bi-weekly or as needed to ensure the highest quality health care delivery

Attends clinical case conferences and residential staff meetings

Teaches Medication Training class to all non-licensed staff per OPWDD curriculum

Responsible for ensuring medical services availability across CCFCS programs

Collaborates with Nursing Services Manager – OASAS and RNs to ensure cross coverage of Nursing staff agency wide

Meets regularly with the Associate Vice President of Nursing

Provides RN coverage to two IDD houses as back up to Part Time RN

Provides back-up RN coverage to residences as needed to ensure compliance with OPWDD regulations

Utilizes benchmark data outcomes and QA reports to maintain a cost effective, dynamic, and high-quality healthcare delivery program

Attends Incident Review Committee, reviews incident reports daily and responds with appropriate staff re-education or training.

Facilitates OPWDD Nursing Incident Review Subcommittee for medication error resolution

Monitors staff medication training and certification status utilizing Medication Compliance tracker

Provides required implementation, oversight & follow-up for corrective action plans.

Participates in staff meetings and clinical supervision as directed

Attends Committee meetings as requested

Operationalizes CCFCS initiatives within the Nursing Team

Participates in relevant Agency meetings and/or trainings

Attends the monthly All Managers meeting and Leadership development sessions

Actively pursues development of professional competencies related to job role performance through reading, supervision, in-service training and attendance at conferences and workshops.

Awareness of and active support of the Agencys Mission, Vision, Values and Strategic Plan. Including, but not limited to, supporting the Agencys Core Competency initiatives of being Collaborative and Community Focused, Innovative and Entrepreneurial, and Culturally Competent and Diverse.

Qualifications

Education: Bachelor’s Degree in Nursing required

Credentials: Valid NYS Dep of Education Registered Professional Nurse License required

Experience: Fiveyears of experience providing Nursing services to individuals with mental health, substance use disorder, and intellectual or developmental disability related services preferred.
