Fairview Range is looking for a Registered Nurse to join our care team at the Fairview Mesaba Clinic in Hibbing, MN.

The RN Care Coordinator focuses on the needs of the patient and their family as they access care from the health care home. The RN serves as a resource and educator to the patient and their family, and coordinates care efforts for a safe, effective, efficient and patient centered transition along the health continuum. The RN provides pro-active disease management using evidence based practices; acts as a liaison between patient/family, medical staff and others involved in treatment (ie ER, Hospitals, nursing homes, home health care and the community at large) promoting patient/family-centered care based on jointly determined health care goals and priorities as defined in the individual care plan; and coordinates implementation of health care home improvements (patient registry, care planning, health outcome data management). As a member of the care team, the RN Care Coordinator supports quality patient and family-centered care principles through performing a variety of high standard functions within a care team to support the needs of the healthcare team and patients/families.

Schedule: This position will work 72-80 hours per two weeks. The shift length will be 8-hours on the day/evening shift, weekends as needed. This position will primarily cover Urology. You will be primarily based in Hibbing, but may provide coverage in Nashwauk and Mountain Iron, as needed.

Care team includes Physicians, Advanced Practice Providers, RN Care Coordinators, RNs, LPNs, MAs, Patient Access Coordinators

As a Registered Nurse CC, you will be responsible for:

Identifying particular needs while implementing quality standards of nursing care in area of clinical specialty.

Evaluating and documenting status and following patient progress through phone calls, test results, and report preparation.

Evaluating quality of nursing care via patient rounds.

Serves as a liaison between patient/family, medical staff, and others involved in treatment. Advocating for patient-centered care grounded in collaboratively resolved healthcare goals and priorities.

Delivering direct patient care around specialty.

Functioning as a resource to patient care units, outpatient clinics, and medical staff regarding clinical specialty.

Providing education services to patients and their families and other hospital staff regarding clinical specialties.

Required Qualifications:

Graduate of an accredited school of nursing.

Current Minnesota license to practice professional nursing.

Specific job requirements will be based on position needs for specialty areas such as anticoagulation, and chronic diseases.

Preferred Qualifications:

Three years clinical nursing experience.

Two years of experience doing care coordination.

Baccalaureate Degree in Nursing.

Health Care Home experience
