Community Health Worker – CHS Pediatrics – Full Time

POSITION SUMMARY
The Community Health Worker (CHW) serves as a link between underserved communities and existing community resources. Through home/office visits and telephonic contacts, the CHW assists clients in overcoming barriers to health, social services, education, and employment by providing connections to community resources and health education tools.

ACCOUNTABILITIES

Follows Northwest Ohio Pathways protocols for completing checklists, pathways, assessments, interventions, follow up, documentation, and care coordination for assigned clients; provides timely and accurate data entry into appropriate database to verify completed pathways, resource connection status and resolutions.
Interviews clients with problems such as personal and family adjustments, health, finances, employment, food, clothing, housing, utilities, personal safety, and physical, emotional, and mental impairments and completes the appropriate checklist or assessment.
Continuity of Care: coordinates with other team members when working with shared clients and provides client reports and updates to providers (Physician, Advanced Care Practitioner).
Determines client eligibility for services such as financial/public assistance, insurance, and social service programs and connects client/family with community resources to meet identified needs, reduce social and health disparities, and promote self-sufficiency.
Medical Support: reviews care plan developed by clinical/medical professionals and performs appropriate follow-up activities as directed; secures information such as medical, psychological, and social factors contributing to client’s situation, and forwards information to medical/clinical professionals for evaluation.
Advocates for client and acts as a liaison between the client and other service providers.
Advises client individually, in family, or other small groups regarding plans for meeting needs and aids client to mobilize inner capacities and environmental resources to improve social functioning.
Meets case productivity goals established by Northwest Ohio Pathways and ProMedica leadership.
Connects clients to Centering program.
Performs other duties as assigned.

Education: High School Diploma or GED (High School Equivalence Diploma).
Skills: Must have the ability to communicate to a variety of diverse individuals in the community. Enthusiasm for life-long learning and professional development. The ability to advocate for disadvantaged populations and to understand and be respectful of an individual’s values, culture, and beliefs. The ability to restrain from imposing one’s own personal values on others. Must have reliable transportation to entire service area. Must be able to understand directions and communicate and respond to inquiries; effective verbal, interpersonal, and written communication skills; ability to work effectively on an interdisciplinary care team. Must be able to use computerized documentation system and laptop computer. Maintains knowledge of community resources in all service areas.
Years of Experience: Successful completion of CHW internship in a health care setting.
Certification: A current and valid community health worker certificate issued by the board of nursing obtained within one year of hire.

PREFERRED QUALIFICATIONS
Education: Associate degree in a health or social service career field.
Years of Experience: 2 or more years community health worker experience.
