Job Title: Mental Health Practitioner Wellstone Job Classification: Non-Exempt Supervisor: Program Supervisor Wellstone Location: Wellstone Benefits: Including but not limited to: Medical, Dental, Life, 401k, PTO Job Summary: The Mental Health Practitioner provides direct support services, including skills training, crisis assistance, treatment plan development, medication administration, group facilitation, and 1:1 interventions to ensure an optimal experience for the people we serve. The practitioner works closely with the rest of the team to collaborate on progress towards treatment goals. These positions are based in a short-term residential facility and shifts consist of days, afternoons, midnights, weekends, and holidays. Duties/Responsibilities: Conduct intake assessments, completing documentation within the electronic health record within required timelines Conduct individual and group skills training to develop and enhance stability, competencies, personal/emotional adjustments, and independent living, activities, and community skills Develop and update treatment plans for all assigned people Coordinate care and provide referrals for services to support clients in reaching and maintaining their optimal level of functioning Act as a client advocate and assist them in obtaining and maintaining benefits Provide crisis assistance, including de-escalation, as needed Complete prior authorizations as needed for services; coordinate with the RMHC billing team as needed Support, coach, and assist clients with completing activities of daily living Attend required staff meetings, multidisciplinary staff meetings, and training Provide program transportation as needed Conduct self in a manner that reflects knowledge and integration of cultural competencies Properly report all instances of suspected abuse or neglect to the program manager and follow requirements for mandated reporting Review policies and procedures regularly and seek answers to questions via communication with the direct supervisor or another representative of RMHC in authority Follow all Federal guidelines under the Department of Labor s statutes and laws and any other regulatory agencies. Other functions as needed to perform assigned duties. Required Skills/Abilities: Philosophical approach consistent with the Range Mental Health Center Valid driver s license, good driving record, and reliable transportation Flexibility to meet demands in in a fluid and changing environment Sufficient command of the English language to be able to effectively communicate both verbally and in writing Non-judgmental attitude toward persons with disabilities. Ability to be flexible, to work as a team member, and to interact effectively with the people we support, employees and other professionals Good understanding of local and state mental health system Strong time management ability Education and Experience: Qualify as a Mental Health Practitioner as defined by the Minnesota Dept of Human Services. (4-year human service degree plus 2000 hours supervised mental health experience, or a master s degree in human services, or 4,000 hours supervised mental health experience) Physical Requirements: Adequate level of vision to use computer Sufficient hearing to accurately record information received verbally Ability to climb stairs Ability to sit and to stand for extended periods of time Ability to lift up to twenty-five (25) pounds Ability to use a computer keyboard
