Responsibilities

Palliative care provides care for patients and their families who are experiencing advanced, life-threatening illness and helps them live as well as possible through excellent symptom management, advance care planning and psychosocial support. The Palliative Care Nurse Practitioner or Physician Assistant works collaboratively with the Palliative interdisciplinary team and consultant to deliver medical care to hospitalized patients. The focus of the job includes providing acute hospital care to complex medical patients with a high symptom burden, ranging from patients who are undergoing treatment with curative intent as well as those who may be at the end of life. The NP or PA provides comprehensive assessment, ordering, conducting, and interpreting diagnostic tests, diagnosis, prescription of pharmacologic agents and treatments and non-pharmacologic therapies. The NP or PA will work with the interdisciplinary care team in coordinating discharge planning and facilitate the patients transfer to the next phase of care. The NP or PA will consult and make referrals to other specialties and members of the health care team as appropriate. During the night shift there is a physician on call to help with complex patient’s available by pager and cell phone.

Qualifications

Nurse Practitioner: Masters prepared graduate of an accredited school of nursing and Nurse Practitioner program. Current Minnesota RN and APRN license and certified as an ANP, ACNP or FNP.

Physician Assistant: Graduate of accredited Physician Assistant program with current PA certification by NCCPA and registration as a PA by the Minnesota State Board of Medical Practice.

Certified or eligible for certification. BLS certification required. ACLS recommended.

Prescriptive authority should be obtained within 2 months of hire. Acquisition of DEA number is recommended for all positions within 2 months of receiving prescriptive authority.

Additional Qualifications:

Hospice and Palliative Medicine certification highly encouraged within 2 years of hire.

Strong written and oral communication skills are required and must also have demonstrated skill in working collaboratively with all members of the healthcare team.

Schedule Details

This position requires a rotation of days, nights, weekends, and holidays. Day shift length is 8 or 12 hours and night shift is 13 hours. This is a 24/7 service area. There will also be cross coverage responsibilities with the palliative consult service on occasion.

Weekend Schedule

Every 4th weekend and six weekends per year on-call to cover in the event of an acute absence. 1 to 2 holidays per year on a rotational basis. It is a rotating schedule position and nights are an expectation (approximately 30-40%).
