POSITION SUMMARY
Our In-Office Case Manager primary responsibilities include but are not limited to:
Review of RNs OASIS Assessments to Ensure Completion and Accuracy
Record Case Information and Complete All Necessary Forms with Accuracy
Assist with Case Management including: Draft Service Plans, Review Case Progress, Determine Case Closure
Facilitate Multiple care aspects including: Case Coordination, Information Sharing, Discharge
Ensure Requests for Visits are Filled Out and Sent to Appropriate Staff
Follow-up on Patient Cases
Oversee All Patients to Ensure Care is Initiated in an Efficient and Timely Manner
Review Clinical Records to Ensure Services Have Been Provided in Accordance with the Patients POC
Ensure Physician Orders are Signed
Communicate with the PCP and/or Insurance Co. Regarding Changes in Condition, the Need for New/Modified Orders, etc.,
Meet with Direct Care Staff and Administrative Staff to Communicate any Follow-ups or Concerns in Regard to Patients
Hold and Document Case Conferences with other Members of the Healthcare Team
Assist with Verifying Notes For Billing
Verify Appropriate Documentation Resides in Patients Clinical Record
Develop Effective Working Relationships with Clinical Team Throughout Lifespan of Case Management Process
Ensure all Deadlines are Met
Adhere to Professional Standards as Outlined by Protocols, Rules, and Regulations
Abide by the Agencys Code of Conduct
KNOWLEDGE, SKILLS, EXPERIENCE REQUIRED
Must Have a Valid RN License (State of Florida)
Minimum of 2 Years Home Health Experience
OASIS Home Health Background
Possess a Good Level of Computer Literacy
Experience in Microsoft Office (Word, Excel, Outlook, and PowerPoint)
Knowledge of Medicare Guidelines
Authorization Experience
Must Have Excellent Organizational and Management Skills with Close Attention to Detail
KNOWLEDGE, SKILLS, EXPERIENCE PREFERRED
BSN or MSN
3-5 years Experience in Home Health
