Options Case Manager

East Tennessee Human Resource Agency, Inc.

Duties/Responsibilities:

– Complete in-home assessments to determine eligibility for the Options for Community Living and National Family Caregiver Support programs to help individual remain independent in the community.
– Develop care plans based on client needs.
– Provide care support services to clients after enrollment.
– Monitor and update care plans over time to best serve the client.
– Enter care support data in a dedicated client tracking system.

Qualifications/Requirements:

– Graduation from an accredited four year college with a B.S. Degree in Social Work, Psychology, Gerontology, Sociology, Counseling, Nursing, or related field; or
– Graduation from an accredited four year college with a B.S. Degree not in a related field and with a minimum of 2 years experience working with older persons and/or adults with disabilities ; or
– Minimum of completion of 2 years of accredited college or university and 2 years experience in the field of social work or related field.
– Must have good organizational, interpersonal and decision making skills
; or
– Any equivalent combination of education and/or experience.
– Valid Tennessee Driver license.
