Case Advocate
Family Centered Services
The Case Advocate is responsible for coordinating and collaborating with internal programs, community agencies, and resources to advocate for at-risk children and families directly and indirectly in order to facilitate critical needs assessments, plan goals, and progress monitoring services for referred families.
This position will have direct contact on a regular and continuing basis with children and families who are receiving services from FCS.
Job Type: Full-time
Salary: $15.00 – $16.00 based on experience and level of education.
RESPONSIBILITIES:
Referred Family Advocacy and Support:
– Advocate for and provide support to referred families in order to meet and exceed individual family progress goals.
– Gain insight and information about referred family needs and their community environment in order to lead families through the Family Centered Services goal-setting process.
– Provide various home-based services and resource connections to at-risk families.
– Initiate referrals and connections with internal programs and local community support and services.
– Provide transportation to families, as needed, to attend programs and appointments to meet their goals.
– Develop and maintain familiarity with community resources and maintain relationships with social workers and agencies, including school systems and other various institutions, related to the family.
– Efficiently and thoroughly complete required documentation as required by supervisor.
JOB REQUIREMENTS
To be successful in this role, you need to have the requisite knowledge, skills, and abilities:
Bachelors degree in social work, psychology, sociology, or a directly related human service field and 2 years of experience in social service for children or families.
Associate degree in a related human services field with a minimum of three years experience working directly with families in a human service field.
Experience in child development.
