Job Summary
The Case Manager coordinates campus support systems and resources to students involved in the behavioral intervention process. As a member of the CARE Team, the Case Manager assists the CARE Team Chair with overall case management by triaging referrals, coordinating interventions, tracking students’ completion of recommendations. They work closely with other campus resource providers, clinicians, student affairs units, and community support systems in coordinating a response that balances the needs of individual students with the safety and welfare of the University community.
Essential Duties and Responsibilities
Support an on-going caseload of students who benefit from a systems approach to student success, wellness and retention.
Triage referrals, schedule and coordinate interventions, assist in care plan development.
Includes follow up with students to ensure compliance with recommendation and provide support and consultation to the faculty and staff involved when students of concern are referred.
Provide timely communication with CARE Team members and other university administrators regarding students of concern and update case progression in the electronic database.
Manage all data, functions and information in the Maxient database related to students of concern and maintain adherence to guidelines for disclosure and access.
Assist with campus-wide trainings on identifying and responding to students of concern, creating a community of care, and goals of the team for individuals and the university.
Design and implement outreach efforts targeting students, faculty, and staff, including presentation and marketing materials.
Work closely with Counseling and Psychological Services, Apartment and Residence Life, and various campus resources and other relevant partners.
Minimum Qualifications
Master’s degree from an accredited institution in social work, higher education and student affairs, counseling, public health or related field.
Previous experience working with a behavioral intervention team or students of concern
Understanding of the application of FERPA, medical privacy guidelines, and other applicable laws and policies.
Preferred Qualifications
Understanding of mental health terminology.
Direct experience working with traditional/non-traditional aged college students.
Utilization and knowledge of the Maxient database system or other electronic case management system.
Knowledge, Skills and Abilities
Maintains knowledge of university policy and procedures, student development and counseling theories, and trauma informed practices.
Ability to assess services and learning outcomes.
Ability to identify problems, use sound judgment and reasoning to make crucial decisions autonomously.
Knowledge of current trends in alcohol education and risk reduction practices.
Ability to take initiative and to balance multiple priorities.
