Social Worker II – Family Caregiver Support Program

The duties listed below are not all of the duties that may be assigned but are those that are considered as essential for an employee to perform.
– Complete assessment: required paperwork/determine eligibility of client, focusing on: the environment, support systems, mental/physical health, abilities/disabilities, goals and finances. Assist in determining supplies/medical equipment that may be beneficial for the client/care recipient and/or respite service options. Purchase and delivery of supplies to client/CR.
– Make referrals and link respite agency with client/care recipient. Fax/mail paperwork to agencies/client during the time that the client/care recipient is receiving respite services. When notified that there is an issue/concern, follow up with client/care recipient or agency to insure issues/concerns are quickly and effectively resolved.
– Identify psychological stressors of client/care recipient and respond appropriately, which may include: emergency intervention, education/training and/or stress reduction management.
– Complete day sheets recording daily work using corresponding service codes and complete monthly reports. Complete narratives explaining in-depth correspondence between social worker and client/care recipient and/or other involved persons. Update required paperwork for client annually/submit to supervisor for review.
– Terminate clients/care recipients that have passed away, no longer eligible or wish to terminate services.
– Provide information and emotional support to caregivers including participants in the monthly Caregiver Support Meetings. Facilitate meetings with meal site participants and other groups to share information regarding FCSP services.
– Compute monthly cost of service (supplies or respite) for each client who states that he/she are above poverty level and prepare and mail a consumer contribution letter to the client. Copy and submit receipts for all purchases.
Minimum Qualifications:
1. Masters Degree in social work from an accredited school of social work
2. Bachelors degree in social work and one year of experience in the area of case management, assessment and referral, supportive counseling, intervention, psycho- social therapy or treatment planning
3. Bachelors degree in a human service field with at least 25 semester hours of approved coursework and two years of experience in a related area
4. Bachelors degree and three years of related experience.

Social Work Assessment and case management skills needed, as well as knowledge of medical and mental health issues/terminologies, knowledge of geriatrics/aging issues. Computer skills required.
A valid drivers license is required.
