North central Pennsylvania has an increasing population of individuals 65 years of age and older. Moreover, this population has an increased burden of chronic disease as well as social isolation. Most counties in the service area of Geisinger Medical Center are above the “PA Living Alone Benchmark” as well as the “PA Chronic Condition Benchmark”. This can lead to increased rates of hospitalization and risk of poor outcomes during and after hospital admissions. A specialist in geriatrics to assist in the care and outcomes of hospitalized patients can positively impact inpatient length of stay, medication reconciliation, quality & safety outcomes, and readmissions.
Position Summary
Interviews patients to obtain information such as symptoms, medical histories, social history, and family histories. Performs and/or interprets diagnostic tests as needed.
Collaborate with inpatient medicine and surgical teams in order to provide safe, quality and efficient care for geriatric patients in the inpatient environment.
Provide specialized geriatic medical consultation for perioperative surgical patients: this may include medication reconciliation, daily consultation, assistance with disposition planning and family engagement.
Develop protocols specific to geriatric populations in order to improve clinical and patient experience outcomes for geriatric patients in the Central Region.
Educate clinical staff (nurses, social workers, care managers, advanced practitioners and physicians) on a recurring basis about specific challenges associated with the care of inpatients of advanced age.
Facilitate educational activities in geriatrics for learners including medical and advanced practitioner students, residents and fellows.
Diagnose and design a plan of treatment for injuries and/or illness. May prescribe or administer medications. Prepares, maintains, and reviews records that include patients histories, examination findings, treatment plans, or outcomes.
