Set on a 36-acre campus in Commack, NY, the Gurwin Healthcare System includes the 460-bed Gurwin Jewish Nursing & Rehabilitation Center, providing short-term rehabilitation and long-term skilled nursing care, memory care, respiratory and ventilator care, palliative and hospice care, social and medical adult day programs, and on-site dialysis and infusion therapy. Sharing the campus is Gurwin’s Assisted Living community (Gurwin Jewish ~ Fay J. Lindner Residences), our Certified and Licensed Home Health Agency, and our independent living community (Fountaingate Gardens).

We are currently seeking an On-Call Certified Occupational Therapy Assistant who will embody the passion and commitment required to deliver the ultimate care for our residents. We are looking for someone with daytime availability Monday through Friday.
EDUCATION AND EXPERIENCE REQUIREMENTS:
Current New York State Licensure or eligibility.
REQUIREMENTS INCLUDE:
Provides treatments to the resident in accordance with the goals and plan set by the Occupational Therapist. Modifies or adjusts treatment within the limits of the plan of care based upon resident’s reactions.
Progresses treatment under the supervision of the Occupational Therapist.
Independently organizes a caseload that reflects the department’s productivity standard.
Maintains appropriate record keeping in accordance with facility and regulatory guidelines.
Participates in the Inter/Intradepartmental In-service Education Program.
Acts as Clinical Instructor, providing education and supervision to COTA students.
Maintains a working knowledge of job duties and departmental policies and procedures. Makes recommendations for revisions.
Actively participates in the departmental Performance Improvement Program.
Assists in the planning, development or revision of programs within the Rehabilitation Department.
Participates in resident/family/discharge care planning conferences.
Performs other related duties as requested by the Supervisor.
Demonstrates an attitude of cooperation and professionalism when dealing with peers and supervisors.
Assists in the transportation of residents as appropriate.
Adheres to all aspects of Customer Service, some examples include the following: working together, showing empathy, courteous communication, respect of others.
