Social Worker – Orthopedic and Transitional Care Unit

Job Summary:

In this position, the social worker is assigned to provide coverage at the Upstate Medical University Community Campus, Transitional Care Unit and Orthopedic Unit. The social worker assigned a medical inpatient units assists with throughput and capacity. The social worker provides comprehensive psycho-social assessment, intervention, management, support, and assesses for aftercare needs of the at-risk patient population. In addition, the social worker addresses social determinants of health, complex care needs and works collaboratively with the interdisciplinary team to recommend and deliver appropriate social work intervention and support. Upstate Social Workers perform other relevant duties as assigned including back-up coverage and support for other social workers to meet operational needs.

Minimum Qualifications:

M.S.W from a Social Work program accredited by the Council on Social Work (CSWE); M.S.W., L.M.S.W., L.C.S.W. or eligible, 3 years relevant post-masters experience (preferably in a hospital and/or healthcare environment) required. New M.S.W. graduates who are L.M.S.W. eligible will be considered for appointment to entry level (SL2) position at a commensurate salary after obtaining a Limited Permit. Incumbent must obtain L.M.S.W. within 12 months of receipt of limited permit.

Work Days:

Monday-Friday, 8:30 am -5:00 pm, occasional on-call/holiday coverage on rotation based on operational needs.
