Responsibilities include, but are not limited to, helping clients with daily activities, accompanying them to doctors appointments, cooking and serving meals, performing housekeeping duties, running errands and providing support. Job Summary/Objective:
Direct Support Professionals work as part of the professional team responsible for helping individuals with disabilities enjoy full, active, meaningful lives. The Direct Support Professional works hands on with persons served, encouraging community involvement and supporting the individuals to take control of their lives and to achieve their goals.
Essential Duties and Functions:
Support individuals to take as much control as possible over the routines and activities in their home.
Teach, assist and support individuals in the community and at home so that the individual is able to learn, practice and master skills that enable him/her to enhance his/her status by assuming socially valued roles.
Know and follow emergency home procedures for fire, tornadoes, accidents, and other serious incidents affecting supported persons or staff.
Support each individuals existing relationships and increase their network of friends and acquaintances.
Assist persons served upon admission with personal items and familiarizing them to the home.
Ensure persons served are dressed appropriately. Assist them into and out of bed and/or wheelchair as needed. Assist with daily hygiene needs (including personal care, brief change) and routine daily cleaning of personal space, if needed.
Provide a well-balanced diet prepared to meet the individuals needs (when applicable), following all instructions given by the Physician and Health Care Coordinator (Program designated nurse).
Assist supported persons in shopping, meal preparation and clean-up.
Implement the Individual Plan of Service/Person Centered Plan and strategies as written and at the frequency stated in the plan. Collect data accurately. Consistently follow written plan instructions, House Rules and intervention strategies as instructed.
Ensure that the health and well-being of individuals are met by accompanying individuals on appointments as needed, administering medications (or supporting them to self-medicate) in accordance with the Person Centered Plan, Licensure guidelines, DCH guidelines and the policies and procedures of Bay Human Services.
Maintain cleanliness of the home through daily, weekly, monthly cleaning duties, including laundry, dishes, bed making and vacuuming. Offer opportunities for individuals to participate, learn and increase independence.
