Care Coordinator RN- Pain Therapy

Overview

The RN Care Coordinator position at the FV Pain Management Center has a variety of nursing responsibilities including, procedural support, clinic visit involvement, phone triage management, specialty care education and process improvement! the individual in the position will work with a multidisciplinary health care team to coordinate the treatment process of the patient and family as they manage their pain; and to enhance program growth and development

Responsibilities Job Description

Coordinates care for patients and families in collaboration with the multidisciplinary team across the continuum of care. Manages the process for a proactive approach to pain management, which includes self-reliance and self-care.

Serves as clinical guide/resource to chronic pain patients and their families, the health care team and the community.

Maintains communication with referring health care providers regarding a comprehensive plan of care for patients and family.

Coordinates the development, implementation, and evaluation of multidisciplinary education plan. Apply the pain management philosophy of self-care management when providing education to patient, family, and community.

Provides phone consultation and triage to patients, families, other healthcare professionals and the community.

Supports in clinic procedures, procedure monitoring, post procedure education and discharge.

Qualifications

Required

Education

Associate degree RN in nursing

Experience

One year work experience as an RN

License/Certification/Registration

Current MN RN license

Preferred

Education

BSN or BS/BA of nursing

ACLS

Experience

Three year’s RN experience in Pain Clinic

License/Certification/Registration
