Exercise Physiologist – Cardiac Rehab – Per Diem

Position Summary:
The incumbent shall be responsible for providing quality care to medical and surgical cardiovascular patients of cardiac rehab services by supporting the development and implementation of a comprehensive Cardiac Rehabilitation Phase I ‐ III program that involves the patient and family. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement, problem solving, and productivity enhancement in a flexible interdisciplinary manner.
Education, License & Cert:
Prefer graduate from an accredited four ‐ year Bachelor’s degree program in Exercise Physiology / Physical Education or related field. However, may consider coursework in exercise physiology with direct patient care experience in lieu of Bachelor’s degree.
Experience:
Education/experience in developing safe exercise prescription and education for the cardiac patients is preferred.
Essential Functions:
Communicates and demonstrate a professional image/attitude for patients, families, clients, coworkers, and others.
If applicable, conduct and document in EPIC the initial Phase I review to include medical record review and patient interview to effectively communicate education for risk factor modification based on individual risk factor history. Determine appropriateness of Phase II referral.
Provide appropriate Phase I‐III exercise prescription resulting in positive outcomes as indicated by quality monitoring and patient satisfaction.
Evaluate and document patient response to treatment. Patient response to the treatment plan is evaluated by patient achievement of goals with update of goals as needed. Collaborate with Cardiac Rehab nurse. Provide information to patient/family regarding status, program planning and discharge plans throughout the rehabilitation program.
Document the course of patient care including progress made to justify need for ongoing intervention. Follow documentation guidelines for the inpatient and outpatient Cardiac Rehab environment according to the department policy/procedure/guidelines.
Provide patient and family education on risk factor modification and exercise guidelines. Integrates family members into the treatment, educational and discharge planning process for a safe transition back to home. Educational methods are adapted to meet patient and family education needs.
Participating in continuing education/professional development activities. Attends/presents at departmental in‐service sessions as scheduled. Attends external professional growth activities as scheduled and communicates information to the team.
Adhere to policies and practices of applicable federal and state guideline, and entity specific policies. Is familiar with and acts according to contents of applicable professional code, employee/handbook, unit Policy & Procedure and organizational policies.
Meets established organizational productivity standards within a fiscally responsible manner.
Participates in performance improvement and quality related initiatives and uses data to improve quality of care delivered.

The pay for this position ranges from $18.55-$29.40 per hour
