POSITION TITLE: Targeted Case Management

SUPERVISOR: Director of Operations

Education, Experience, and Skills Required :

Bachelor’s degree in one of the behavioral sciences or related fields including, but not limited to, social work, psychology, or nursing from an accredited college or university.

Essential Functions :

1. The development of a written personal service plans

2. A regular review and revision of the written personal service plan with the recipient and the recipients legal representative, and others as identified by the recipient, to ensure access to necessary services and supports identified in the plan;

3. Effective communication with the recipient and the recipients legal representative and others identified by the recipient;

4. Coordination of referrals for needed services with qualified providers

5. Coordination and monitoring of the overall service delivery to ensure the quality and effectiveness of services;

6. Assistance to the recipient and the recipients legal representative to help make an informed choice of services

7. Advocating on behalf of the recipient when service barriers are encountered or referring the recipient and the recipients legal representative to an independent advocate

8. Monitoring and evaluating services identified in the personal service plan to ensure personal outcomes are met and to ensure satisfaction with services and service delivery

9. Conducting face-to-face monitoring with the recipient at least twice a year

10. Completing and maintaining necessary documentation that supports and verifies the activities in this section;

11. Coordinating with the medical assistance facility discharge planner prior to the recipients discharge into the community

12. A personal service plan developed and reviewed at least annually with the recipient and the recipients legal representative. The personal service plan must be revised when there is a change in the recipients status.

REQUIRED PHYSICAL ABILITIES:

1. While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear.

2. The employee is occasionally required to sit and climb or balance.

3. Must be able to sweep, mop, shovel, and lift up to 40 lbs.

4. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:

1. Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.

2. Must be able to perform basic operations on a computer within the Windows operating system.

3. This position requires problem solving skills, critical thinking skills, and the ability to multi-task.

WORK ENVIRONMENT:

1. May be exposed to communicable diseases when working in a health care environment.

2. The employee is occasionally exposed to wet and/or humid conditions and outside weather conditions.

3. The noise level in the work environment is usually moderate.
