This RN will coordinate the OPAT Program in support of the OPAT Medical Director and through the collaborative guidance and supervision of a designated ID Pharmacist (PharmD). Second year Infectious Diseases (ID) Fellows are physician members of this OPAT Team, working under the supervision of the OPAT Medical Director and the ID Fellowship Program Director. The OPAT Nurse will utilize standard nursing practices to provide support and care for patients receiving outpatient IV antimicrobial therapy, while focusing on the needs of the patient and exemplifying the service excellence goals of care, concern, respect and cooperation. The OPAT Nurse will act as the liaison between patients, physicians, home health agencies, nursing homes and other agencies to coordinate OPAT care for patients. As a contributing member of the OPAT Team, this nurse will assist in developing and implementing the patient treatment plan, while recognizing and applying knowledge of Loyolas 14 Magnet Forces.

Patient Care & Assessment: Functions in the role of patient advocate by being in the position of initial contact for patient-related issues, especially during the transition from inpatient to outpatient care. Clarifies and implements physician orders by ordering testing and treatments as prescribed by the physician. Recognizes symptoms and establishes priorities consistent with patients condition. Documents patients physical and emotional status and adds to the plan of care in order to address patient needs appropriately.

Plan of Care: Reviews and evaluates plan of care, obtains information from patient and/or family, and adds to plan of care as appropriate (Magnet Force #2, 6). Collaborates with the multidisciplinary team, the patients, and family to promote efficient patient care (Magnet Force #10, 11, 13). Provides for the physical, cultural, and psychosocial needs of the patient and family. Integrates evidence-based practice and professional standards whenever possible. Serves as a champion for patient safety by continually observing and monitoring the clinical environment and assuring that safety goals and standards are considered environment and assuring that safety goals and standards are considered when providing care in the practice setting.

Communication: Performs required documentation of laboratory values, changes in orders and communications in a competent and effective manner to meet patient and/or family needs and regulatory requirements. Protects confidentiality of patients Personal Health Information (PHI). Magnet Force #1, 7, 10).

Discharge Planning: Reviews and evaluates discharge plans with the multidisciplinary team, the patient and family. Ensures patients have follow-up and weekly laboratory values ordered as recommended when ID signs off. Arranges follow-up Infectious Diseases appointments, when necessary, ideally with ID Fellow or Attending who saw the patient in the hospital.

Quality Improvement: Utilizes time and resources effectively to promote improvement in quality and cost outcomes (Magnet Force #1, 7, 10).

Professional Development Activities: Demonstrates accountability for professional growth and job performance by seeking opportunities to enhance job skills and knowledge (Magnet Force # 4, 14).

Position Requirements:

Education: Bachelors degree in nursing required

Experience: Less than one year of previous job-related experience required; 1-2 years of previous job-related experience preferred

Licensure/Certifications: Basic Life Support (CPR); Current Registered Nurse license State of Illinois required
