Memorial Health System seeks a Physical Therapist in Forsyth, IL.
Duties:

1. Maintains flexibility, organizes and prioritizes time to accommodate
assigned patient load.
2. Meets departmental standards and insurance/reimbursement guidelines
for documentation of care.
3. Daily notes
4. Communicates with doctors
5. Recertifications
6. Evaluations
7. Billing
8. Outcome measures
9. Evaluates patients; plans and delivers care and treatment to
progress patient to goals agreed upon by patient, family and
Physician.

1. Re-evaluates and plans programs on an ongoing basis.
2. Competently applies all Physical Therapy modalities.
3. Instructs patients and families in home programs.
4. Suggests changes in patients clinical approaches, and secures
orders as appropriate.
5. Communicates and collaborates with rehabilitation team.
6. Maintains existing equipment and recommends purchase of new
items as consistent with Hospital and Department Strategic Plan.
10. Maintains current professional knowledge.
11. Attends all mandatory educational programs and continuous education
required for licensure.
12. Seeks continuing education to improve quality of care.
13. Maintains fiscal responsibility of the Department and the Hospital.
Compliant and accurate implementation of billing process.
14. Actively looks for ways to reduce costs.
15. Maintains patient/family confidentiality to comply with HIPPA
regulations.
16. Provides in-services and public education functions.
17. Supervises students, Physical Therapy Assistants, and aides.
18. Serves on committees as requested.
19. Performs other duties as assigned by Director, Managers, and Team
Leader.
20. Recommends and implements new programs as directed and as they
pertain to Hospital and Department Strategic Plan.
21. Competency to complete OASIS assessment accurately and completely.
