Job Summary
Under the general guidance of the practice physician, the Clinical Coordinator-Thoracic Oncology is primarily responsible for maintaining standards of care and patient satisfaction, and for coordination and provision of proficient care of pulmonary and thoracic oncology patients.

Frye Regional Medical Center, located in the beautiful foothills of North Carolina is a 355-bed acute care hospital, 81-bed Behavioral Health facility and more than 70 primary and specialty care providers.
Essential Functions
Obtain medical histories and identify pulmonary symptoms; perform and record vital signs
Obtain social history
Review and schedule all new patient referrals
Work with patient/family and provider team to develop/institute plans post procedure needs
Collaborate with patients, families, physicians, and other healthcare providers to develop, implement, and evaluate comprehensive care plans, ensuring optimal patient outcomes.
Serve as the primary contact for the same-day diagnostics and surgery program, coordinating care and assisting patients in scheduling surgeries, while liaising with various services and physicians to ensure all necessary procedures and consents are obtained.
Educate patients on pre-operative medications and communicate any concerns to physicians, striving to enhance the overall patient experience through effective communication and support.
Provide post-operative education to patients on home care and disease management, fostering continuity of care by establishing relationships with regional providers and facilitating referrals to outpatient programs.
Act as a resource for care providers regarding quality measures, ensuring compliance with guidelines and assisting in daily care plans and issue resolution.
Screen postoperative telephone calls, offer solutions or connect patients with community resources, and provide post-discharge support to prevent readmissions.
Assist in medication reconciliation, ensuring a safe medication regimen, and educate them on medication compliance and understanding.
Demonstrate professional accountability, adhere to organizational standards, and pursue personal growth and development goals.

Minimum Education
High school diploma or equivalent required
Certifications
Current credentialing (certification/registration). Must maintain credentialing with American Association of Medical Assistants (AAMA-CMA), NRCMA, NHA, NCCT or American Medical Technologists (AMT-RMA).
Minimum Work Experience
A minimum of 5 years’ experience in the healthcare setting. Requires excellent clinical, critical analysis, communication and organizational skills. Working knowledge of excel, PowerPoint and Word. Must be able to function independently within the limits of the position. Strong collaboration and interpersonal skills, with the ability to work effectively within a multidisciplinary team. Knowledge of standard of care treatment for Pulmonary and thoracic oncology patients
