$3,000 hiring bonus for eligible external candidates that meet required qualifications and conditions for payment.
This position requires traveling in Orange county.
The Care Transition Associate performs a variety of patient-centered administrative tasks to support care coordination within acute care hospital hubs. This care transition work is focused on post-acute care needs for patients requiring home health, hospice, home infusion and home medical equipment. Support patients and families with their specific discharge plan and plan of care for home health and hospice referrals. The Care Transition Associate supports the work of business development by ensuring a high level of customer service and connection between our patients and referral sources increasing the volume of patients accessing care through Providence post-acute ministries, including home health, hospice/palliative care, home infusion, home medical equipment and other referrals.

Required Qualifications:
Associates Degree or High school diploma or equivalent plus three years of work experience is equivalent to the Associates degree
Demonstrated work experience with patients and families.
Work experience, preferably in health care setting, providing customer service and administrative support to professionals.
Preferred Qualifications:
Coursework/Training: Medical terminology.
Coursework/Training: Health care training.
Coursework/Training: Educational courses in healthcare.
MS Word and Excel software programs.
Experience with Epic electronic medical records (EMR)

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.

About the Team
Providence Home and Community Care (HCC) is a service line within Providence with over 8,000 caregivers and 2,000 volunteers serving nearly 200,000 people a year, in their homes, in clinics and in a variety of congregate living ministries across our seven-state footprint.
HCC provides a full range of services and support for people of all ages, especially those facing chronic or life-limiting illnesses. Our core competencies include restorative care, longitudinal care and care through the end of life. Our service lines include assisted living/skilled nursing/rehabilitation, home health, home infusion/pharmacy services, home medical equipment, hospice and palliative care, Program of All-Inclusive Care for the Elderly (PACE), personal home services/private duty care and supportive housing.
