We are devoted to creating and supporting services that remove barriers to services in the community, rather than in emergency rooms and institutional settings. .

Responsibilities:

Provide supportive stabilization services to clients/families in the community in need of crisis supports and community stabilization.

Provide side by side interventions to individuals/families that focus on problem solving, skills training, modeling behavior, optimism and encouragement.

Provide interventions in the office, virtually, in the community and in client’s residences based on client preference, client need, and at the clinical direction of the urgent care team.

Respond to community evaluations, treatment intervention support, urgent care needs as assigned in conjunction with Stabilization Clinicians and other members of the urgent/triage team within CBHC.

Provide and support mobile crisis interventions, stabilization, planning and evaluations.

Provide follow up and follow along supports as part of short-term treatment planning, stabilization and urgent care.

Complete NOMS assessments as assigned, aspects of clinical assessments, and other documentation and required to meet CBHC/Eliot standards.

Provide short term case management supports to individuals including: managing income, insurance, other entitlements, accessing other services and community resources and scheduling/coordination of all medical and psychiatric appointments.

Support warm handoffs from triage, stabilization and urgent care to CBHC teams.

Assist clients who arrive at the Community Based Location seeking treatment such as crisis intervention services, follow up services, or admission to CCS/YCCS.

Complete all necessary paperwork, documentation and insurance verification within the established timeframes.

Enter and maintain daily documentation, notes in electronic medical record / computer tracking system for all clients who are seen for services, call the crisis lines or are receiving services.

Qualifications:

Requires a Bachelor’s Degree in a Human Services field

One year of experience with adults, families or children with severe and persistent mental illness and/or emotional disturbances

Must have a strong commitment to recovery-oriented services.

Must have skills to establish supportive relationships with persons with severe and persistent mental illnesses, kids, families and respect for their preferences for treatment.

Must have ability to communicate clearly and effectively with individuals in rehabilitation and treatment. Must have ability to provide flexible services that are community based and focused on team work.

Ability to serve as a role model and assist clients of varied backgrounds achieve daily goals and structure.

Ability to work both independently and as part of a team. Ability to multitask and think calmly in a fast paced, often chaotic environment.

Schedule: Monday – Friday 9a-5p

Compensation: $25.48/hour
