RN Assistant Manager, Home Care

Atlantic Health System

Responsible for utilizing the nursing process to assess, diagnose, plan, implement and evaluate the plan of care for patients served. Collaborates with all members of the health care team with a focus on promoting evidenced based care within a healing culture for the patient population served and their families.

Principal Accountabilities:

1. Works with interdisciplinary team members to assess and establish plans of care.

2. Collaborates with all members of the health care team with a focus on promoting evidence based care within a healing culture for the patient population served.

3. Mentors field staff in all aspects of caring for patients.

4. Ensures current proficiency of knowledge regarding Medicare COPs and Joint commission standards.

5. Performs other related duties as assigned.

Required:

1. Bachelors degree in nursing or related field.

2. 2-4 years of experience in Home Health Care, hospice or relevant area.

Preferred:

1. State Licensure for Registered Nurse.
