Earn $22.17/hour with the selection of the Enhanced Pay Option. 
Position Details 
Under the direction of the Blended Case Management Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan.  As a member of the BCM team this position will serve as primary contact for no more than 30 individuals in the community.  Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients needs along with supportive counseling, and will include on-call responsibilities.

The ideal candidate will possess the following qualifications: 
 High school diploma OR AT LEAST 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science required. 
 Bachelors degree preferred. 
 AT LEAST 2 years experience in public or private human service, with 1 year being direct care required. 
 Valid Drivers license required. 
 All clearances required by OMHSAS regulations.
