Summary:

The Clinical and Scheduling Operations Assistant provides administrative and clinical support to teams within home care. The Clinical and Scheduling Operations Assistant is responsible and accountable for accessing appropriate information systems and referral sources to obtain additional information, contacting physicians to obtain/confirm orders, contacting patients/families to provided telephonic initial contact, supports the clinical team in the scheduling of home care visits, and assists team managers in coordination of care.

Responsibilities:

Interacts with patients to ensure initiation and coordination of homecare services, manages clinical concerns and symptoms, and supports the virtual implementation and clinical follow-up of the plan of care.

Answers phones in a timely manner. Manages/handles patient requests and routes appropriately, enlisting management assistance when appropriate. Retrieves voicemails in a timely manner, takes accurate and thorough messages and routes appropriately through EMR, and identifies opportunities to improve the patient experience as per practice/department protocols and/or measurements.

Schedules patient appointments via phone by determining reason for visit, following established schedules and protocols, using appropriate billing area/appointment location, communicating changes and confirming appointments, and, as needed, offering alternative and canceling/rescheduling appointments. Provides responsive and proactive telephonic patient care including care coordination with specialists, ancillary services, hospitals, labs, home care agencies and all other involved health services.

Collaborates with clinical team, schedulers, and leadership to ensure scheduling coordination and quality patient care, including notification of scheduling delays, order changes, etc. Provides support for ensuring Penn Chart reports are communicated to the clinicians for completion and follow-up.

Reviews appointment schedules and updates contact types to maintain accurate schedules for the purposes of regulatory compliance such as therapy functional assessments, home health aide supervisory visits, recertifications and discharges.

Performs clerical functions in Penn Chart to ensure deficiencies in patient records are addressed for timely billing. Monitors patient’s arrival in different entities (i.e.: ED, inpatient status) and updates home health schedules for accuracy and communicates with clinical team. Gathers, reviews and ensures receipt of essential components of the patient’s chart including consents, NOMNCs, and other regulatory documents.

Maintains complete and accurate work queues and responds to work queue issues timely.

Maintains clinical competency and able to perform home visits as necessary.

Credentials:

LPN-PA or LPN-NJ (Required)

Req Pennsylvania and/or New Jersey LPN licensure depending on geographic location and scope of responsibilities. LPN

Education or Equivalent Experience:

H.S. Diploma/GED (Required)

Education Specialization:Graduate of an approved Practical Nursing ProgramEquivalent Experience:•And 2 years Clinical Healthcare experience with significant interactions with patients, clinicians, and/or customers.
