Make a Difference

The Patient Transfer Center Registered Nurse Case Manager is responsible for the assessment, planning, implementation, coordination, monitoring and evaluation of services across the continuum of care to ensure quality patient outcomes and appropriate utilization of health care services. Responsible for all future direct admissions and lateral transfers within Community Health Network. To provide ongoing project planning and role development. perform duties and tasks as needed by the Network/ Transfer Center. Identify and bring forward any Financial and Insurance issues. Support Transfer Center workflow daily, assist Transfer Center Specialist, and be in attendance to work related meetings. The Patient Transfer Center is located at 13050 Parkside Drive in Fishers, IN.

Exceptional Skills and Qualifications

Graduate of National League for Nursing, Accreditation Commission for Education in Nursing or Commission on Collegiate Nursing Education accredited school of nursing.

Licensed as a Registered Nurse by the Indiana Professional Licensing Agency – (required).

Two years of recent clinical experience or three years recent hospital case management experience- (required).

Certification in Case Management (CCM or ACMA) – (preferred).

Team building skills with the ability to function independently and interdependently as a member of a network team.
