Registered Nurse Case Managers deliver direct care to patients in their homes, whether its at home or in a care facility. Nurses provide more than just medical assistance; they also listen attentively to patients needs and honor their wishes. One of the most important aspects of being a Registered Nurse Case Manager is building trusting relationships with patients and their families. This trust allows us to serve as the central point of communication within our interdisciplinary team of holistic care providers.

Registered Nurse Case Manager (RN) Qualifications:
6 months minimum prior skilled nursing experience.
An active unencumbered state Registered Nurse license.
Current drivers license and auto liability coverage.
Interest and Ability in providing patient care in homes and facilities.
Registered Nurse Case Manager (RN) Benefits & Schedule:
Monday to Friday, 8:30 AM – 5:00 PM
No Mandatory On-Call.
