Job Title: Integrated Case Manager for Case Management / Discharge PlanningJob Description
We are seeking an experienced Integrated Case Manager to manage and coordinate the discharge planning of patients from the hospital to their home or another medical facility. You will create and implement patient discharge plans from an office computer, primarily supporting a specific unit but possibly assisting others as needed. This role involves conducting comprehensive assessments, developing personalized care plans, and facilitating seamless transitions between care settings. The case load is typically a 1:25 ratio but may vary. Responsibilities include assessment, planning, facilitation, care coordination, evaluation, and advocacy to promote patient safety, quality of care, and cost-effective outcomes.
Hard Skills

This is a Contract-to-Hire position with a duration of 13 Week(s).
Work Site
This is a fully on-site position in Detroit, Michigan.
Work Environment
You will work at a computer/desk all day in an office unit with about 20-25 case managers. The environment is fast-paced, and you will be counted on to complete your case load to avoid putting additional work on the rest of the team. The schedule is M-F, 8:30am – 5:00pm, with rotating weekends every 4-6 weeks. Weekend shifts are scheduled in advance, and you will receive time off during the week if you work a weekend. You could be working on any of the medical floors, including Cardio and ICU.
