Job Description

Our client is looking for Case Managers across the state of Florida. This role will provide case management support for members enrolled in client health plans to promote timely community services delivery. You will be visiting members and conducting comprehensive assessments and then helping coordinate the appropriate visits, contacts, and referrals to community and social programs to address the members needs. Additionally, you will document all communications and care updates accurately and ensure all reporting is handled within the given requirements. The role will require roughly 75-80% travel for field visits with 20-25% remote for administrative responsibilities. This is a permanent role and the ray range is from $26-30/hr.

Skills and Requirements

Bachelors Degree

2 years minimum of case management experience

Direct IDD experience

Must be willing to travel within their region (mileage will be reimbursed) Bilingual in Spanish null
