As an Admissions Nurse, you are the first touchpoint for our patients, ensuring they start their journey to recovery on the right foot. This specialized role is dedicated to supporting patients specifically discharging from Inspira Hospitals. Your expertise will be key in preventing hospital readmissions, enhancing patient satisfaction, and allowing more patients to receive their post-acute care in the comfort of their own homes. By collaborating closely with transitional care managers, hospital discharge planners, and other healthcare professionals, youll build strong relationships that will help our patients safely meet their medical needs without entering a facility.

Your Role and Responsibilities:
Patient-Centered Care: Create designated care plans tailored to each patients needs, making home visits in Cumberland, Gloucester, and Salem counties.
Clinical Expertise: Perform essential duties, including medication administration, wound care, treatments, and client assessments, while closely monitoring their conditions and documenting your findings.
Specialized Support: Work closely with transitional care teams, discharge planners and population health to support patients as they transition from Inspira Hospitals to home healthcare, ensuring seamless and safe care with positive outcomes.
Technology-Driven: Utilize state-of-the-art touchpad tablets for accurate documentation, ensuring efficient and effective patient care.
What You Bring to the Table:
Credentials: A current NJ RN license and at least one year of recent clinical experience. Prior home care experience or comprehensive medical assessment experience is a plus, but not required.
Independence & Interpersonal Skills: Ability to work independently, manage time effectively, and communicate well with patients, families, and healthcare partners. Let your smile be seen and felt!
Tech-Savvy: Solid computer skills, with prior experience in electronic medical records (EMR) preferred.
Mobility: Willingness to travel to assigned cases in your designated territories.
